New Patient Registration Procedure

We are happy to register new patients who reside within the PRACTICE AREA. Unfortunately we cannot register patients who are outside our practice area due the travel distances involved should a patient require a house call from a GP. Please contact the practice by telephone, or come into the surgery, where our receptionist will happy to help you find out if you live within our practice area.

You will be required to make an appointment with the nurse.

If you are on regular medication, you will require an appointment to see the doctor.

You may choose the service of any of the doctors in the practice.

Procedure

If you would like to register as a patient with us, we require you to complete 2 registration forms. One form is an NHS form and the other is our new patient registration form. You can collect all the forms from our reception or download them from the links below. Once completed please bring them along to the practice with 2 forms of ID, one with an address and one with a photograph. (eg a utilities bill  and a passport).

Once the forms are completed, along with all the appropriate identification information, our receptionists will arrange an appropriate appointment with our Practice Nurses. If you are on regular medication, the nurse will refer you to a GP.

There have been occasions where patients have simply not turned up for their initial appointment with our nurse. However we understand that personal circumstances change, and re-arranging an appointment will not be a problem.

NHS Form – this form is required by the NHS to ensure that your medical records are moved from your previous practice. (Download NHS Form here)

Practice Form – We use this form to collect any contact details and medical information that will give us a complete record to help provide you with the best possible service. (Download Practice Form here)
